Dr. Keith Blasingame

Dr. Keith Blasingame, President and Founder of ProActive Physical Therapy Centers, PLLC, has been committed to excellence in patient care since 1994. After graduating from the University of Oklahoma in 1994 with a degree in Physical Therapy, Keith was employed as a staff therapist by a large orthopedic group in Fort Worth, Texas.

Dr. Keith Blasingame, President and Founder of ProActive Physical Therapy Centers, PLLC, has been committed to excellence in patient care since 1994. After graduating from the University of Oklahoma in 1994 with a degree in Physical Therapy, Keith was employed as a staff therapist by a large orthopedic group in Fort Worth, Texas. Within one year, he came to the Houston area to be the Site Coordinator at this group's satellite clinic. In only 9 months, he was able to positively impact the lives of people in the community by serving 120% more patients than when he arrived.

In 1996, Keith took on a new challenge and helped open a Sports Medicine Facility affiliated with a large Houston area hospital. After 4 ½ years in which Keith maintained a relationship with that community, he decided it was time to become an entrepreneur.  So, on August 14, 2000, he opened ProActive Physical Therapy’s Pearland Center.

Keith's vision for ProActive Physical Therapy Centers is to offer the community a clinic that is free to make decisions that will benefit the people in the local community. Keith offers numerous programs to benefit the patients of ProActive Physical Therapy Centers. Just a few of the programs are Sports Medicine, TMJ/headache specialty program, Aquatics, Work Hardening, and Job Analysis.

Keith stays very involved with the local communities in which he owns clinics.  He has been a Board member of Coppinger YMCA since 2003.  Past positions held include:  Board member of Pearland Chamber of Commerce, Board member of Pearland Rotary, Board member Project Joy and Hope, President of Pearland chapter of BNI, Membership Committee member of Pasadena chapter of BNI, member of steering committee for Texas Invitational basketball tournament, elected King for 2004 Pearland Neighborhood Center, and member of Fastbreak Booster Club in Pearland.  It is through these affiliations that Keith feels he is able to make the biggest impact on giving back to the communities.

Susan Ballard

Integrator/VP of Finance and HR

Susan Ballard officially joined the ProActive team in 2018 as our Payroll Administrator although she was already part of our family as a massage therapist for over 15 years. Currently, Susan serves as our Integrator (think COO with a 21st century twist) and VP of Finance and HR. She considers herself a Southern Yankee as Houston is home, but her roots as an east coast girl are never gone. Susan and her husband have three boys and two grandsons. Together they are active in the Boy Scouts of America/Scouts BSA with all three boys being Eagle. They enjoy traveling and spending time outdoors. Prior to family, Susan spent many years in corporate america in various roles within the commerical real estate industry. Her mission is “To help people improve their quality of life.” Being part of the ProActive team allows her to continue that mission through patient care as well as building a culture for our employees where they grow and enjoy what they do.

Amy Hudak

Amy and her family moved to Texas from Connecticut in 2005. A native of the East Coast, she completed her Bachelor of Science degree in Physical Therapy at Quinnipiac University in 1988. Amy has been a practicing physical therapist for more than 27 years, with 25 of those spent in outpatient orthopedic settings. Amy and her family moved to Texas from Connecticut in 2005. A native of the East Coast, she completed her Bachelor of Science degree in Physical Therapy at Quinnipiac University in 1988. Amy has been a practicing physical therapist for more than 30 years, with most all of those spent in outpatient orthopedic settings. Specializing in manual therapy techniques, aquatics, and therapeutic exercise with a concentration on core strengthening, Amy has been a part of the ProActive Team since 2009, 1st as the director in Clear Lake, then director in Pearland, and most recently moving into her current role as VP of Operations. Amy hopes to use her many years of experience to help guide the clinical operations at each location and empower the staff to provide the outstanding patient care ProActive is known for.

When she’s not at work, Amy pursues her love of singing as part of her church Worship Team (though she’s been known to bust out in song at work, too!) She also enjoys spending time with her family, walking her dogs, reading, and exercising (she’s a fan of running and kickboxing workouts). Amy is also a "Mamie" (grandma) to 2 beautiful granddaughters, and she treasures every moment she can spend with them!

Amy’s goal is to support the amazing team of healthcare providers at each of the ProActive locations and assist in making every patients experience both effective and FUN!

Dr. Lisa Sellers-Simon

Dr. Lisa Sellers-Simon
Clear Lake Clinic Director - PT, DPT,MTC, MSPT

Dr. Lisa Sellers-Simon, PT, DPT,MTC, MSPT, is a native Houstonian.  She has lived in Europe, NYC and New Orleans, but came full circle to her hometown in 2001.  She graduated top of her class at City University of New York -- magna cum laude and commencement speaker for her graduating class of 1987.  She then pursued a career in acting with her most unusual job as co-star of an Off-Broadway Splatter Rock Musical with a cult following!  Upon leaving the acting field, she pursued a career in fitness.  As a personal trainer in NYC, she wrote feature articles for Harper’s Bazaar and Glamour Magazine.  Later she would attend Texas Woman’s University for physical therapy.    For nearly 20 years, Lisa has worked in a variety of clinical settings.  She has loved it all for the joy she receives in helping others achieve a higher quality of life thru the rehabilitation process.  Her greatest passion is orthopedics and she is delighted to have come full circle once again.   She employs a variety of manual therapy techniques which includes trigger point dry needling. She emphasizes the importance of good posture and core strength and wants patients to feel at ease while reaching their maximum functional potential.   Outside of work, she is with her family, taking spin classes, or crafting.  She and her husband love teaching children’s Sunday school.

Dana Warren

Dana Warren
Physical Therapist - PT

Dana is a native born Texan from Denton, TX. She attended Texas Woman’s University to get
her BS in Physical Therapy, graduating in 1980. She began her career in Dallas, TX and when she
married in 1982, she moved to Clear Lake to join her husband who worked with a NASA
contractor.
She began her career at a pediatric OP clinic in Dallas and when she came to Houston, she
worked at Hermann Hospital at the Texas Medical Center and remained there for 3 years. She
worked in two school districts for 19 years, home health for many years, and then a variety of
settings while doing Traveling therapy.
She has one son and two daughters who all attended Clear Lake High school. She now has a
grandson and granddaughter who live in Connecticut with her daughter-in-law and son who is a
Chief in the Coast Guard. They were a major draw for her to do several years of Traveling PT
work which took her to Washington state when they were stationed there. She also worked in
Arizona, Louisiana, and Missouri, as well as several places in Texas. She has returned to her
home in Clear Lake to complete her career as she now has a 3 month old granddaughter here in
Houston and both of her daughters and their spouses live in the area.
Dana loves to travel, spend time with family and friends, read, and has many craft interests
such as crosstitch, smocking, scrapbooking, etc.
Dana’s goal is always to provide each individual with whatever they need to get the best results
for an overall positive rehabilitative experience.

Earl Williams

Physical Therapy Assistant - PTA

Earl is an experienced Physical Therapist Assistant that has returned to ProActive Physical Therapy Centers, after providing home physical therapy for the past few years. With nearing two decades of experience, Earl works to provide the best care to his patients as they manage to return to their highest level of function. Earl utilizes his knowledge and skills not only to help patients overcome their current impairments, but to educate his patients in prevention of injury with return to activities of daily living. Prior to his initial tenure with ProActive from 2004 to 2011, Earl attended Houston Community College Systems where he received his Associate Degree for Physical Therapist Assistant in 2004. April Royall

April Royall
Physical Therapy Assistant - PTA

Born and raised in the Houston area, April Royall is a state licensed Physical Therapist Assistant. She earned her Associate of Applied Science in 2007 from San Jacinto College and has been with ProActive Physical Therapy since 2009, and has been a valuable member at our Clear Lake, Pearland, and Pasadena clinics. 
Over the past 15 years she has gained extensive experience working with a wide variety of orthopedic conditions,  including acute and chronic conditions and taping to improve function. Her passion is the rehabilitation process of post op knees and shoulders, and rehabilitation of young athletes with the goal of returning to sports!
In her free time she enjoy spending time with her husband and two children. They love participating in sports and traveling together.

Sametria joined the team at ProActive Physical Therapy Centers in September 2023 just after completing
her Doctor of Physical Therapy degree from Thomas Jefferson University in Philadelphia, Pennsylvania.
Sametria is a native-born Texan and enjoys spending her off time with family and friends. Her
compassionate nature will have you on the road to recovery in no time!

Frederick “Erick” Manalo

Erick joined the team at ProActive Physical Therapy Centers in April of 2023. With over 20 years of
experience, he is a strong healthcare professional with a Doctor of Physical therapy degree focused in
Medical Clinical Sciences/Graduate Medical Studies from Utica College of Syracuse University. A
resident of the Manvel, Texas area, Erick enjoys spending time with his wife and children during his time
off.

turn their wounds into wisdom and empower them with hope and optimism for a more compelling
future to a new life for themselves and their kids.
